To take a maternity leave, you will need to complete a Pregnancy Confirmation Form (below) with your physician and submit to HR as well as a Leave Request Form (available at MTI, your school or HR) and submit to HR. See the info sheet below as well. Contact MTI if you wish for further assistance.
