You will need to complete a Leave Request Form (available at MTI, your school and HR) and turn it in to your principal. Your principal will sign and forward the form to Human Resources at the Doyle Administration Building.

You will also need to obtain a statement from your physician which indicates that you need to take a medical leave. This statement should include a beginning date for your leave and an approximation of the amount of time you will need to be off of work. Your physician will then need to provide a second statement when you are released to return to work at which time your physician should indicate whether or not there are any restrictions placed on your return. These physician statements should be sent to Human Resources with a copy sent to MTI.
